Can a judge make a decision on a case or motion if there is a conflict of interest?

If the presiding judge worked at the same law firm with one of the counsels involved with the motion or case, or having direct ties to that case or motion, is that a conflict of interest. And if it is, if they did not recuse themselves due to a conflict of interest is there something one can do after a judgment has been entered?
